The dietary manager also stated that the facility dietician visits the facility and is to sign and approve any menu substitutions on the substitution log. The cook reported preparing toast instead of biscuits on 04/06/2026 because the facility was out of biscuits. The regional dietician confirmed that facility practice requires any menu substitution to be documented on the substitution log and reviewed/signed by the dietician, and that such documentation should have been present for the substitutions made on the identified dates.
