Summary

Surveyors identified a deficiency related to medication administration when a resident did not receive Levothyroxine Sodium 25 mcg as ordered by the physician. The resident had a standing order, dated 09/30/2025, to receive one Levothyroxine Sodium 25 mcg tablet every morning. The facility’s EMR showed the medication was reordered on 03/30/2026, and the contracted pharmacy’s Consolidated Delivery Sheet, signed by an LPN, indicated the Levothyroxine had been delivered to the facility at midnight on the documented delivery date. However, review of the resident’s April 2026 eMAR revealed no documentation that the Levothyroxine dose was administered on 04/04/2026 and 04/05/2026 by the LPN. Progress notes entered by the LPN on 04/04/2026 and 04/05/2026 stated that the Levothyroxine was “on order” and that the resident was informed the medication was still on order. The resident reported not receiving Levothyroxine all weekend because staff told her the facility was out of the medication and expressed concern that it had not been reordered before it ran out. In an interview, the LPN confirmed she did not administer the Levothyroxine on those two days. The pharmacy technician confirmed that the LPN had signed for the delivery of the Levothyroxine, and the DON stated that if the medication was available in the facility, it should have been administered as ordered on those dates.
