Summary

The facility failed to follow dietician-approved menus and to document and obtain dietician approval for menu substitutions on two of three observed days. Review of the dietician-approved Week 1 menu showed biscuits were to be served for Monday breakfast, and the Week 5 menu showed mock pecan pie was to be served for Thursday lunch dessert. The menu substitution log for 03/13/2026 to 04/08/2026 contained no entries for substitutions for the lunch menu on 04/02/2026 or the breakfast menu on 04/06/2026. On 04/02/2026, the posted lunch menu listed pecan pie as dessert, but at least 37 residents in the dining room were observed receiving a cake-like dessert with white icing instead of the mock pecan pie specified on the Week 5 menu. A resident later reported not being served all of the items that were on the facility’s menu for a dinner meal on 04/03/2026. On 04/06/2026, observation of breakfast service showed residents in the dining room were served toast halves instead of the biscuits listed on the Week 1 Monday breakfast menu. The contracted dietary manager stated that Week 5 menus had been used the prior week and Week 1 menus were in use for 04/06/2026 through 04/12/2026, confirmed that mock pecan pie could no longer be obtained and that a different dessert had been served on the Thursday in question, and acknowledged that the substitutions for the 04/02/2026 lunch dessert and 04/06/2026 breakfast bread item were not documented on the menu substitution log. The dietary manager also stated that the facility dietician visits the facility and is to sign and approve any menu substitutions on the substitution log. The cook reported preparing toast instead of biscuits on 04/06/2026 because the facility was out of biscuits. The regional dietician confirmed that facility practice requires any menu substitution to be documented on the substitution log and reviewed/signed by the dietician, and that such documentation should have been present for the substitutions made on the identified dates.
