A resident on hospice care with a CVA diagnosis had a PRN order for lorazepam 0.5 mg PO q6h for anxiety. Facility policy required meds to be administered as prescribed. A CNA informed an RN that the family requested lorazepam, but the RN deferred administration to coincide with the resident’s next scheduled meds instead of giving it when requested. The family later directly told the RN the drug had been requested earlier and not given; the RN replied it could be given now and made an inappropriate comment about the resident possibly dying. Documentation showed the lorazepam was not administered until later that afternoon, even though giving it at the time of the initial request would have been within the ordered PRN interval.

The deficiency involves the facility’s failure to follow a physician’s order for an as-needed (PRN) medication and to provide treatment in accordance with the resident’s needs and preferences. Facility policy required medications to be administered as prescribed and in accordance with written physician orders. The resident had a physician order for a hospice consult and was admitted to hospice with a diagnosis of cerebrovascular accident. The resident also had a physician order for lorazepam 0.5 mg by mouth every 6 hours as needed for anxiety for 14 days. Facility records showed that lorazepam had last been administered on September 27 at 9:50 a.m., and the ordered PRN dosing interval allowed for administration at the time it was requested by the family on September 29. On September 29, during report at 3:00 p.m., a nurse aide informed RN Employee E3 that the resident’s family had requested lorazepam for the resident. Employee E3 told the aide that the lorazepam could be given with the resident’s 4:00 p.m. scheduled medications, rather than administering it when requested. At 3:10 p.m., the resident’s representative directly approached Employee E3, stating that lorazepam (Ativan) had been requested at 2:30 p.m. and had not yet been given. Employee E3 responded that he/she would give the medication then if the representative wanted, and added, “but if he/she is dead in an hour don’t blame me.” Documentation on the Controlled Drug Receipt Record/Disposition Form showed that lorazepam was administered at 3:35 p.m., confirming a delay in providing the PRN medication despite the family’s earlier request and the fact that administration at that time was within the prescribed timeframe.
