Health deficiencies cited at Preferred Care At Absecon during CMS and state inspections, most recent first.
The facility failed to maintain a clean and safe environment on the 2nd floor and [NAME] Hall, with surveyors observing hair and stains on medication carts, mechanical lifts, and linen carts. The DON acknowledged the issues and noted the lack of a cleaning policy.
The facility failed to make survey results readily accessible to residents and visitors. Residents were unaware of the location of the survey results, and staff struggled to locate the results book, which was eventually found in an inaccessible area. Additionally, the survey results available were outdated, with the last available results dated 2009.
A surveyor observed that a refrigerator/freezer in a resident pantry was unplugged, leading to unsafe food storage temperatures. Items in the freezer were not frozen, and the refrigerator was too warm, compromising food safety. An LPN explained that the unit was unplugged to use the microwave, resulting in the deficiency.
Facility Fails to Maintain Clean and Safe Environment
Penalty
Summary
The facility failed to maintain a clean, safe, and sanitary environment in two of its units, specifically the 2nd floor and [NAME] Hall. Surveyors observed several deficiencies, including hair tangled in the wheels of Medication Cart #2 and a mechanical lift on [NAME] Hall. Additionally, a clean linen cart showed blue, tan, and brown stains, and the covers for the clean linen carts were in poor condition with rips. On the 2nd floor, similar issues were noted with hair and a yellow substance on the wheels of the medication and treatment carts, as well as brown stains on the front of the medication cart. The mechanical lift's wheels also had pieces of plastic and hair caught in them. During an interview, the DON, ADON, and LNHA were informed of these environmental concerns. The DON stated that environmental services are responsible for cleaning the medication carts and mechanical lifts according to a monthly schedule, and emphasized that hair and plastic should not be present. She also noted that clean linen cart shelves must be free of stains and inspected by all staff, and that the covers should not have rips. However, the facility was unable to provide a policy outlining the cleaning procedures for these items.
Survey Results Accessibility Deficiency
Penalty
Summary
The facility failed to make survey results readily accessible to residents and visitors, as evidenced by multiple observations and interviews conducted by surveyors. During a resident council task, five long-term residents were asked about the location of the most recent state survey results, and all five residents reported being unaware of where these results were located. This indicates a lack of communication and visibility regarding the survey results for the residents. Further investigation by the surveyors revealed that the Licensed Nursing Home Administrator (LNHA) and the Regional Nurse (RN) were unable to immediately locate the survey results book, which was eventually found behind the receptionist desk, inaccessible to residents and visitors without assistance. Additionally, the survey results on the [NAME] Hall unit were placed on a shelf behind a nurse's station, inaccessible to residents, especially those in wheelchairs, due to locked doors. The survey results available were outdated, with the last available results dated 2009. Similar issues were observed at the first-floor nurse's station, where staff were initially unable to locate the survey results book.
Improper Food Storage Due to Unplugged Refrigerator/Freezer
Penalty
Summary
The facility failed to maintain kitchen sanitation in a safe and consistent manner, which was observed during a survey on the 2nd Floor resident pantry. The surveyor, accompanied by an LPN/UM, found that the temperature log for the resident refrigerator and freezer had not been recorded for the day of observation. Upon further inspection, the internal temperature of the freezer was found to be 42 F, and the refrigerator was at 52 F, both of which are outside the safe temperature range for food storage. The surveyor noted that items in the freezer, such as Cherry Italian Ice and vanilla ice creams, were not frozen, and items in the refrigerator, including bagged sandwiches, milk cartons, and pudding cups, were stored at unsafe temperatures. The LPN/UM explained to the surveyor that the refrigerator/freezer had been unplugged, likely to use the microwave, which required the same outlet. This action led to the improper storage temperatures observed. The LPN/UM then removed and disposed of all food products from the refrigerator and freezer. This incident highlights a lapse in maintaining proper food storage practices, which is crucial to prevent foodborne illnesses.
