The discrepancy was identified when two LPNs conducted a narcotic count and found the medication missing. Despite the facility's policy requiring immediate notification of the DON upon discovering discrepancies, the nurses delayed reporting the issue. The facility's policy also required discontinued medications to be destroyed promptly with two nurses present, which was not adhered to. The pharmacist confirmed that medications should be destroyed on-site as soon as they are discontinued. The failure to follow these procedures led to the unaccounted loss of the controlled medication.
