Health deficiencies cited at Signature Healthcare Of Bowling Green during CMS and state inspections, most recent first.
The facility failed to provide three residents with estimated costs for continued therapy services after their Medicare benefits were exhausted. The SNF ABN Form CMS-10055 lacked the necessary cost information, preventing informed decision-making. The Business Office Manager was unaware of the requirement to include estimated costs on the form.
The facility failed to ensure accurate MDS assessments for two residents. One resident's hospice designation was not reflected in their assessments, despite being in hospice care. Another resident, who communicated using an iPad, was incorrectly assessed as having no speech. MDS coordinators acknowledged these oversights.
Failure to Provide Estimated Costs for Continued Therapy Services
Penalty
Summary
The facility failed to provide three residents with an estimated cost for services when their Medicare therapy services were terminated due to exhaustion of Medicare benefit days. This deficiency was identified during a survey where the facility's Beneficiary Notice of Medicare Non-Coverage policy was requested but not provided. The residents involved were not given the opportunity to make informed decisions about continuing therapy services because the Skilled Nursing Facility Advance Beneficiary Notification of Non-coverage (SNF ABN) Form CMS-10055 did not include the estimated cost for services. The residents affected were admitted to the facility on different dates and had their skilled therapy services discontinued after exhausting their Medicare benefits. The Business Office Manager admitted during an interview that she was unaware of the requirement to fill in the estimated cost on the SNF ABN Form CMS-10055. She mentioned that the residents chose an option where they were not responsible for paying, and she did not think she had ever filled in the cost on the forms.
Inaccurate Resident Assessments in MDS
Penalty
Summary
The facility failed to ensure accurate assessments for two residents, which could lead to their needs not being appropriately addressed. For one resident, identified as R88, the facility did not accurately reflect the resident's hospice designation in the Minimum Data Set (MDS) assessments. Despite being admitted to hospice care on May 20, 2023, this status was not recorded in the Quarterly MDS Assessment dated February 20, 2024, nor in the Annual MDS Assessment dated May 22, 2024. During an interview, MDS coordinators acknowledged the oversight, stating that Section O should have been coded for hospice services. Another resident, identified as R58, was admitted with cerebral palsy and communication deficits, being deaf and nonverbal. The Annual MDS Assessment dated August 22, 2023, and the Quarterly MDS Assessment dated May 14, 2024, both indicated that the resident had no speech. However, the resident communicated using an iPad, a fact that was not captured in the communication section of the MDS assessments. MDS coordinators admitted that the assessments were incorrectly coded, as the resident was able to make his needs known through the provided communication tool.
