Failure to Provide Correct Diet Consistency
Penalty
Summary
The facility failed to ensure that Resident 228 received the correct diet as ordered by the physician. Despite the physician's order for a Regular, Soft Bite-Sized diet, Resident 228 was repeatedly served pureed food items. Observations on multiple occasions showed that Resident 228 was given pureed cornbread, pureed salad, scrambled eggs, ham, meatballs, and green beans, all in pureed consistency. This was contrary to the physician's order and the facility's diet guide sheet, which specified that these items should be served in a soft and bite-sized consistency. Interviews with the Dietary Manager and the Regional Nurse Consultant confirmed that the food items should have been served in a soft and bite-sized consistency as per the physician's order. The facility's policy on General Food Preparation and Handling also indicated that food should be altered to the appropriate consistency to meet individual needs. Despite these guidelines, the facility did not follow the prescribed diet for Resident 228, who has a diagnosis of dysphasia and moderate cognitive impairment, as indicated in her clinical record and care plan.
Infection Control Deficiency
Penalty
Summary
The facility failed to ensure proper infection control procedures for one of the five residents reviewed for resident care. Specifically, an LPN did not change gloves or perform hand hygiene while providing catheter care and bilateral buttocks care to a resident with obstructive and reflux uropathy and chronic kidney disease. The resident's care plan included orders for foley catheter care and treatment for moisture-associated skin damage (MASD) on the bilateral buttocks. During the observation, the LPN donned PPE and began catheter care without changing gloves or performing hand hygiene when transitioning from contaminated to clean areas. The LPN then assisted the resident in turning onto their side and observed bowel incontinence and a red rash on the buttocks. The LPN retrieved clean supplies from the closet and performed incontinent care without changing gloves or conducting hand hygiene. The LPN continued with the bilateral buttocks treatment care, again failing to change gloves or perform hand hygiene. When reminded, the LPN acknowledged the oversight and changed gloves before continuing the treatment. Interviews with the Corporate Nurse Consultant and the Director of Nursing Services confirmed that staff are required to wash hands and change gloves during personal care when moving between contaminated and clean areas. The facility's policy on Standard and Transmission-Based Precautions also mandates hand hygiene and glove changes in such situations.
