The Director of Nursing confirmed that EBP was initiated only during MDRO outbreaks, and there were lapses in ensuring proper signage and PPE availability. The facility's policy on EBP, revised in May 2024, required gowns and gloves for high-contact activities, but this was not consistently followed, leading to deficiencies in infection control practices.
