Interviews confirmed that the facility's elopement policy, which required immediate reporting and resident accounting when an alarm sounded, was not followed. The failure to respond appropriately to the alarm and to notify staff resulted in the resident being unsupervised outside the facility for an extended period.

The DON confirmed that these discard instructions were not adhered to, leading to the deficiency noted in the report.
