The Administrator admitted that it was her responsibility to check for accuracy and to save the PBJ provider validation report after upload. She acknowledged that she did not save the report and accepted responsibility for the deficiency.

During an inspection, it was observed that the medication storage room contained expired items, including 13 Covid tests with an expiration date of 11/10/2023, a tube of nonsteroidal anti-inflammatory cream that expired in 06/2019, and a bottle of eye drops that expired in 03/2022. Additionally, a medication cart on the 300 Hall contained a box of medication used to treat Type II diabetes with an opened pen that lacked an open date, despite the manufacturer's instructions that the pen is good for 56 days after opening. The Assistant Director of Nursing (ADON) stated that expired medications are brought to the locked medication storage room and recorded in a blue book for the pharmacist to pick up monthly for destruction. Further investigation revealed that the Licensed Practical Nurse (LPN) responsible for the medication cart on the 300 Hall checks for expired drugs only once weekly. The facility's Medication Storage and Labeling Guidelines clearly state that drugs should not be kept on hand after their expiration date and that outdated, contaminated, or deteriorated drugs should be immediately withdrawn from stock and disposed of according to drug destruction procedures. Despite these guidelines, the facility failed to adhere to proper storage and labeling practices, leading to the presence of expired and improperly labeled medications in the medication storage room and on the medication cart.
Failure to Perform Hand Hygiene and Cover Clean Laundry

Despite these policies, the observed actions of the laundry aides did not align with the facility's infection control procedures, leading to potential contamination of clean linens and clothing.
