The DON acknowledged she had not recently read the grievance policy and stated that department heads were responsible for completing investigations and notifying the complainant verbally, with no written responses being provided. The ADM, after reviewing the grievance policy and the March grievances, stated the policy was not being followed, noted blank sections on the grievance forms, and confirmed that written follow-up was not being provided to persons filing grievances, despite the policy requiring written responses and written summaries of investigations.
