Summary

The facility failed to ensure that food and drink were palatable, attractive, and served at safe and appetizing temperatures. Resident interviews revealed multiple complaints that meals were cold, with one resident stating she disliked the food and that it was always served cold, another stating the food was cold, and a third reporting the food was usually cold and the plates were not hot. Resident council meeting minutes documented prior concerns that food from the dietary department was cold. A review of the facility’s Resident Tray Assessment form showed that the standard optimal temperature for hot entrées, starches, and vegetables at point of service was greater than 130°F. During observation of a lunch meal consisting of Salisbury steak with mushroom gravy, scalloped potatoes, vegetable blend, and angel food cake, a test tray was prepared and delivered on a meal cart to a resident unit. When the tray was served, food temperatures were recorded below the facility’s standard: the Salisbury steak reached a highest temperature of 109°F, the scalloped potatoes 127°F, and the vegetable blend 109.2°F. The Salisbury steak appeared and tasted dry and was actually a plain hamburger patty topped with gravy, and the scalloped potatoes, made with diced potatoes, were described as bland. Review of recipes showed that the Salisbury steak should have included finely chopped garlic, onions, and peppers mixed with ground beef, eggs, and breadcrumbs, and the scalloped potatoes should have used fresh peeled and sliced potatoes with spices, onions, and parmesan cheese. The plate warmer was observed next to the tray line but unplugged, with plates inside that were not warm. Although a kitchen outlet had been replaced and the plate warmer was known to be incompatible with that socket, maintenance confirmed it could be plugged into another outlet within reach of the steam table and that this had been communicated to dietary leadership, yet the plate warmer was not in proper use at the time of survey.
