Additionally, there were no ICC meeting minutes, no evidence of input from required ICC members, and no documentation of reporting surveillance data, healthcare-associated infection (HAI) rates, or infection control compliance metrics to the ICC. There was also no evidence of an annual infection control risk assessment or development of annual goals and performance measures, demonstrating that the IP did not carry out the required functions of the IPCP.
