The facility failed to follow its established grievance process related to residents’ reports of missing and delayed-return clothing. One resident reported that personal laundry sent out of state took more than a week to return, that some clothing items went missing, and that the facility offered lower-quality replacement items, which the resident refused, leaving the grievance unresolved. Review of the grievance logs showed that while some missing clothing complaints were documented as resolved through reimbursement or replacement, other grievances remained unresolved despite being marked as addressed.
