The deficiency involves the facility’s failure to ensure safe wheelchair transport and adequate supervision, resulting in a resident falling from a wheelchair and sustaining a significant injury. The resident had diagnoses including Alzheimer’s disease, muscle weakness, and a non-displaced type 2 dens (C2) fracture, and was care planned as being at risk for falls and requiring staff assistance with toileting and transfers. On the evening in question, a CNA was transporting the resident to the bathroom in a wheelchair without footrests attached. Imaging identified a type II dens fracture with features suggesting a nonacute fracture, and the resident’s history included a prior C2 fracture in November 2025. When the resident returned to the facility, she had a cervical collar in place and sutures to the left forehead. Interviews with the DON, the CNA involved, and the physical therapist confirmed that facility practice and expectations were that staff should apply wheelchair footrests before transporting residents, and that this was not done at the time of the incident, leading to the fall during transport.
