On the morning of 4/10/26, an LPN stated she was giving the resident’s 7:00 AM and 8:00 AM medications at 9:58 AM, explaining that she had 13 blood sugars to check. At 10:35 AM, the resident reported she had not yet received her pills, nasal spray, eye drops, or inhalers. At 11:04 AM, the LPN returned with a medication cup full of pills, which the resident dumped into pudding and swallowed, followed by administration of nasal spray and an inhaler; no eye drops were observed being given. At 11:07 AM, a white pain patch dated that day with the backing still intact was observed on the resident’s overbed table, and the LPN stated the patch was supposed to be applied at 6:00 AM. The resident reported she usually received her medications late, between 10:30 AM and 11:30 AM, and that her morning medications could be given anytime between 6:30 AM and 10:30 AM. Record review showed the resident’s Medication Administration Record for April 2026 included an order for a menthol topical analgesic gel to be applied to the left arm twice a day as needed for pain, with no administrations signed out for the month. The Medication Administration Audit Report for 4/10/26 showed the resident had multiple medications ordered for 8:00 AM, including oral medications, nasal spray, inhalers, eye drops, and other agents, but the LPN documented administration of these medications between 9:30 AM and 1:56 PM that day. Another resident reported that morning medications could be received between 6:30 AM and 10:30 AM and that the nurse gave her a cup of medications which she kept and took at 9:30 AM without the nurse watching. The DON and an RN both stated that medications are considered on time if given within one hour before or after the scheduled time, and the facility’s Medication Administration Policy required administration within 60 minutes of the scheduled time and observation of resident consumption. The Regional Director of Operations stated that the nurse should watch as medications are taken and should apply any topical medications, and that medications should not be left with the patient to take or apply independently.
