Summary

Surveyors identified a deficiency in the facility’s failure to ensure that physicians reviewed, signed, and dated standing physician orders at the required intervals for three residents reviewed for accidents. Resident #1, who had dementia, dysphagia, anxiety, severe cognitive impairment (BIMS score of 6/15), and was dependent for ADLs, was on an every-60-day schedule for review and renewal of physician orders. Although interim physician orders were signed, the monthly physician orders for this resident were not signed by the physician from September 2025 through April 1, 2026, and the facility was unable to provide the last signed 60‑day physician orders. Resident #5, with dementia, anxiety, dysphagia, severe cognitive impairment (BIMS score of 3/15), and dependence for ADLs, and Resident #6, with dementia, dysphagia, severe cognitive impairment (BIMS score of 1/15), and dependence for ADL care, were also on every‑60‑day schedules for review and renewal of physician orders. For both residents, interim physician orders were signed, but the monthly physician orders were not signed by the physician from September 2025 through April 1, 2026. Interviews with the DNS and Corporate RN revealed that the Medical Director had been signing orders electronically for about six months but was only signing interim/new orders and discharge orders, and that routine standing orders due every 30 or 60 days were not being signed. The Corporate RN stated the software system was configured to trigger only discharged residents’ orders for physician signature, and both the DNS and Corporate RN confirmed that they could not provide signed standing orders, despite facility policy allowing electronic signatures and documenting that time and date of orders are recorded by the computer.
