Summary

The deficiency involves the facility’s failure to implement its Change of Condition Notification policy for a resident who developed a swollen right jaw related to a wisdom tooth. The resident, who had GERD and moderate cognitive impairment and required set-up/clean-up assistance for eating and oral hygiene, had a Change of Condition (COC) assessment completed on 3/5/2026 indicating the jaw swelling and that the physician was notified. Orders were received to refer the resident to a dentist and to monitor the resident for 72 hours for acute changes. A progress note by the Social Services Assistant documented that the resident was referred to the dentist and that the call was transferred to an LVN for orders from the dentist. A subsequent progress note by the LVN documented communication with the dentist, who stated that orders for oral antibiotics would be faxed to the facility as soon as possible. Another RN confirmed there was no documentation of the antibiotic order being transcribed or implemented and no documentation of 72-hour monitoring related to the change of condition. The facility’s Change of Condition Notification policy required the licensed nurse to document pertinent details of the incident, update the care plan to reflect the resident’s status, and document each shift for at least 72 hours, which was not done in this case.
