Summary

Surveyors identified a deficiency in the facility’s failure to ensure adequate supervision and use of assistive devices during mechanical lift transfers. During observation, a CNA was seen operating a mobile floor mechanical lift alone while lowering Resident 2, who was in a sling, into a wheelchair, with no other staff present in the room. The facility’s Accident Hazards/Supervision/Devices policy required assistive devices and equipment to be used according to manufacturer recommendations and staff to be trained on their use, and the DSD confirmed that staff, including CNA 1, had been trained and competency-checked on mechanical lift use.
