Summary

The deficiency involves multiple failures in food preparation, temperature monitoring, and hygienic handling of food and meal plates by dietary staff. During a lunch meal plating observation, five raw chicken patties were individually fried and then placed in a covered pan to await service, but the internal temperatures of three of the five patties were not taken prior to serving, contrary to the facility’s policy requiring temperatures of each pan of product to be recorded before serving. After the patties had been served, the cook acknowledged that he had not taken the temperatures of three patties and stated that all cooked food should be verified to have a safe internal temperature before serving. The Corporate Certified Dietary Manager confirmed that the expectation was to take and record internal temperatures of all cooked food before serving to residents. The Administrator later reported that competency evaluations for the involved dietary staff could not be located. Additional deficiencies were observed in glove use and direct handling of food and plates. During the same lunch meal period, one staff member handed a resident’s plate to another staff member to add a piece of fish; the second staff member took the plate with bare hands, used tongs to place the fish on the plate, then placed the lid on the plate and handed it back without wearing gloves. The staff member later confirmed that gloves are required when handling food and resident meal plates and stated he was in a hurry and forgot to put them on. In a separate observation, a dietary aide placed an onion into a plastic storage bag with bare hands, dropped the onion on a cart, then picked it up with bare hands and placed it in the bag; the aide acknowledged that gloves should have been worn and confirmed that the onion handled without gloves was stored in the refrigerator for use in resident food. The Corporate Certified Dietary Manager confirmed that all kitchen staff are required to always wear gloves when handling any food, cooked or raw.
