Summary

The deficiency involves the facility’s failure to provide food that was palatable, attractive, and served at a safe and appetizing temperature. The facility’s Menu Operational Manual Policy stated that the Dietary Manager would develop menus in collaboration with the Dietitian, considering resident preferences, department resources, and seasonal availability of food. Resident Council Meeting minutes from two separate meetings documented ongoing complaints that food was not cooked properly (either undercooked or overcooked), that condiments such as salt, pepper, ketchup, and mustard were not provided, and that no alternative menu items were available. Two residents reported that their meals were cold and frequently arrived late, and one of them also stated there was not enough staff during lunch. During a test tray observation, the surveyor tasted a meal consisting of meatballs, green peas, and pasta in the presence of the Corporate Certified Dietary Manager. The meatballs were described as pale in color and lukewarm, the peas were lukewarm, and the pasta was tough, bland in flavor, and lukewarm. The Corporate CDM had not requested a meal tray to test for herself. In a subsequent interview, the Corporate CDM and the Registered Dietitian confirmed that neither of them had tested any food served to residents or completed audits of meal temperatures and taste from January through March 2026. This lack of monitoring and quality checks contributed to residents receiving food that was not palatable or served at an appropriate temperature.
