Summary

The deficiency involves the facility’s failure to provide sufficient CNA staffing to meet residents’ daily care needs, including timely call light response and scheduled showers, particularly on Nursing Station 2. Multiple residents reported long waits for assistance, including one resident who waited 30–40 minutes on the toilet, resulting in pain and redness to her legs and bottom. Grievance documentation showed a resident complained that a CNA was not cleaning her thoroughly and that, after requesting another CNA, a licensed nurse told her she would have to wait due to CNAs being busy and then did not return. Resident council minutes over several meetings recorded resident reports of call light wait times of about 45 minutes on all shifts and concerns that CNAs taking lunch breaks two at a time left insufficient direct care staff to answer call lights. Staff interviews and record reviews further demonstrated that resident acuity, including the number of residents requiring Hoyer lifts, was not adequately considered in staffing assignments. A mechanical lift list showed 18 dependent residents requiring Hoyer lifts on Station 2 and 10 on Station 1, for a total of 28 such residents. CNAs reported that staffing was often at three CNAs per station, that call-offs were not always replaced, and that with this level of staffing they could not complete all care, resulting in missed showers and some residents being left wet. One CNA reported that when a CNA left early due to illness, only two CNAs remained on the morning shift, making the shift very difficult and leading to showers not being completed. Shower reports showed multiple residents with scheduled showers on specific shifts had numerous refusals documented when showers were offered on different shifts or late, and the Director of Staff Development confirmed that Station 2, which had the most Hoyer lift residents and more complex residents with behaviors, often had only three CNAs when four were required, and that showers were refused or missed.
