Failure to Develop Care Plans for Residents’ Documented Skin Rashes

Summary

Surveyors identified that the facility failed to develop comprehensive, individualized care plans addressing documented skin rashes for three residents. For one resident with encephalopathy and respiratory failure, the admission record and history and physical documented that the resident lacked capacity to understand and make decisions, and the MDS showed severely impaired cognitive skills with a need for moderate to maximal assistance with ADLs. A Skin Rash Report dated 1/30/2026 documented a rash on both hands, but review of the medical record revealed no corresponding care plan for this skin condition. A second resident, admitted with encephalopathy and chronic kidney disease, was also documented in the history and physical as lacking capacity to understand and make decisions, with the MDS indicating severely impaired cognitive skills and a need for varying levels of assistance and supervision with ADLs. A Skin Rash Report dated 1/28/2026 showed a rash on the upper back. Despite this documented skin issue, the resident’s medical record contained no care plan addressing the rash on the upper back. A third resident, admitted with palliative care needs and chronic kidney disease, was noted in the history and physical as able to make needs known but unable to make medical decisions, and the MDS documented severely impaired cognitive skills and extensive assistance needs for ADLs. A Skin Rash Report dated 9/8/2025 indicated a generalized body rash with complaints of itchiness. Review of this resident’s medical record also showed no care plan for the generalized rash. In interviews, the Infection Prevention Nurse and Treatment Nurse both stated that any resident with a skin issue or rash must have a care plan that includes monitoring, treatments per MD orders, and skin moisture maintenance, and that all nurses are allowed to develop such care plans. The facility’s written policy on care plans stated that goals and objectives are to be entered on the resident’s care plan and reviewed or revised with significant changes in condition, but this was not done for the three residents’ skin rashes.
