Lack of Required IDT (CNA and Physician) Input in Resident Care Plan Meetings

Willowbrooke Court Skilled Center At Manor HouseSeaford, Delaware Survey Completed on 05-06-2025

Summary

The deficiency involves the facility’s failure to ensure that required interdisciplinary team (IDT) members, specifically CNAs and physicians, provided input into residents’ care plan meetings. For three residents, multiple care plan meeting notes lacked evidence of CNA and physician participation or input, despite completed MDS assessments and scheduled care plan meetings. One resident was admitted in May 2023 and had quarterly MDS assessments completed in September and December 2024 and March 2025, followed by care plan meetings on corresponding dates. Each of these care plan meeting notes lacked documentation of CNA and physician input. The SW stated that CNAs are always invited and that she attempts to include CNA input if they cannot attend, but she was unable to provide evidence of such input for the identified meetings. The NHA stated that physician visits should correlate with care plan meetings and that meeting notes should reflect concerns from physician progress notes. A second resident, admitted in August 2024, had significant change MDS assessments completed in September and November 2024 and a quarterly MDS in February 2025, with care plan meetings held shortly after each assessment. Each of these care plan meeting notes also lacked evidence of CNA and physician input. A third resident, admitted in October 2024, had an admission MDS, a significant change MDS, and a quarterly MDS in progress, with care plan meetings held after each assessment. Again, the care plan meeting notes lacked evidence of CNA and physician input. Review of physician progress notes and care plan meeting notes for all three residents showed that physician visits did not occur prior to the care plan meetings and that the care plan notes did not correlate with or reflect physician input. During interviews, the NHA confirmed that physician progress notes did not correlate with the care plan meeting notes and that the expected physician input was not documented.

Failure to Update Care Plans for Comfort Care and Pressure Ulcers
D
F0657 F657: Develop the complete care plan within 7 days of the comprehensive assessment; and prepared, reviewed, and revised by a team of health professionals.
Short Summary

Failure to Update Care Plans for Comfort Care and Pressure Ulcers: The facility did not revise the care plan for a resident placed on comfort care after a clinic visit showed worsening fluid retention, cough, swelling, and decreased strength; the plan omitted the no-hospitalization order, discontinuation of labs, and guidance for comfort if the resident declined. The facility also failed to update another resident’s care plan after the MDS identified four Stage II pressure ulcers, leaving only general skin-risk interventions instead of wound-specific goals and treatment measures.

Short Summary

Care plans and related care guides were not updated for a resident with pain, a resident with recurrent falls, and a resident with severe cognitive impairment and transfer needs. One resident’s plan lacked individualized nonpharmacological pain interventions, another resident’s plan omitted a motion sensor that staff were using for fall prevention, and a third resident’s plan and Kardex incorrectly stated the resident was independent with transfers despite staff using a transfer belt and Hoyer lift with two-person assistance.

Short Summary

Failure to update care plan after change in condition: A resident was hospitalized with acute urinary retention and constipation related to neurogenic bowel, but the care plan was not revised to reflect the new diagnosis or related interventions. Failure to Update Care Plan With Current Diagnoses and Medication Indications
D
F0657 F657: Develop the complete care plan within 7 days of the comprehensive assessment; and prepared, reviewed, and revised by a team of health professionals.
Short Summary

A resident with a history of anemia, moderate dementia, and chronic pain had active orders for aspirin for CAD and sertraline (Zoloft) for depression and chronic pain, but the comprehensive care plan was not revised to reflect current diagnoses and medication indications. The care plan continued to reference anemia and daily aspirin for antiplatelet therapy and included a directive to administer antidepressants for chronic pain without specifying sertraline’s use for both depression and chronic pain. An MDS nurse acknowledged that the resident no longer had an active anemia diagnosis and that the care plan should have been updated to clarify the current clinical rationale for aspirin therapy and the indication for sertraline.
