Residents using wheelchairs had difficulty entering the building, sometimes requiring staff assistance, and one resident reported nearly falling due to the condition of the ramp and door. Staff interviews confirmed that these issues had persisted for some time, and the Maintenance Director acknowledged awareness of some problems but had not completed repairs. The Administrator was not aware of all the deficiencies until they were pointed out during the survey.
