The DON stated she was responsible for monitoring incomplete training modules for nursing staff and said one reason for staff failing to complete training was a breakdown in communication. Facility policy revised 02/2026 stated all personnel must participate in initial orientation and regularly scheduled in-service training, and listed resident rights and responsibilities as a required training topic.
