Staff interviews confirmed residents were expected to receive at least one bath weekly unless they refused, refusals should be documented, and facial hair should be addressed when noticed.

The resident’s medical record lacked an assessment for ADL decline, including any revision to the ambulation care plan.

The facility policy stated that if a resident refuses ADL assistance, staff should ensure the resident is safe, re-approach, and notify the nurse or manager if refusal continues.
