Summary

The facility failed to develop and implement a comprehensive care plan with appropriate interventions for a resident identified as being at risk for elopement. Despite the resident having a care plan in place for elopement risk, the only interventions documented were to redirect the resident as needed and to add the resident's picture to the elopement book.
