The facility failed to follow physician's orders and established protocols for dialysis care for two residents with End Stage Renal Disease (ESRD). For one resident, who had severe cognitive deficits and was unable to make decisions or communicate effectively, the physician's order required removal of the pressure dressing from the right AV fistula 4-6 hours post dialysis if no bleeding was noted.
