Failure to Provide Adequate Towels and Maintain Clean, Safe Shower Facilities

South Pointe Rehabilitation And Care CenterOklahoma City, Oklahoma Survey Completed on 03-03-2025

Summary

The facility failed to provide an adequate supply of towels for bathing, as evidenced by multiple observations and interviews with residents and staff. Residents reported not having towels available for several months, and staff confirmed frequent shortages, with as few as 11 towels available for the entire facility of 182 residents. During multiple walkthroughs, staff and the Director of Nursing were unable to locate sufficient towels in resident halls, shower rooms, or the laundry room, with only a handful of towels found throughout the building. Additionally, the facility did not ensure that shower beds were clean and in good repair. Residents and staff reported that shower chairs and beds were often unclean, with visible debris and substances present. Observations revealed shower beds with smeared substances, debris, and makeshift repairs such as a trash bag holding up a broken undercarriage. Staff stated that cleaning was supposed to occur after each use, but the observed conditions indicated this was not consistently done. Maintenance issues, such as broken shower beds and missing parts, were reported by staff but not always addressed in a timely manner. Further deficiencies included a missing shower hose in one shower room and broken or missing tiles in another, both of which had not been reported or repaired promptly. The maintenance supervisor was unaware of several issues until they were pointed out during the survey, and there was no policy in place regarding maintenance, cleanliness, or towel supply. These failures resulted in residents not having access to a safe, clean, and comfortable environment as required.

Widespread Odors and Environmental Disrepair in Resident Care Areas

Short Summary

Surveyors found that the facility failed to maintain a safe, clean, and homelike environment, with strong, persistent urine and feces odors noted throughout multiple halls and confirmed by staff. On two nursing units, hallways and resident rooms contained torn flooring, food debris, broken blinds, dirty and leaking toilets and sinks, rusted and corroded fixtures, missing outlet covers with oxygen concentrators plugged in, exposed light sockets, unmade and visibly soiled beds, and black, mold-like substances on walls and around toilet bases. Bathrooms had missing ceiling tiles, cracked door facings with brown stains, used briefs and torn toilet paper on floors, and toilets with brown or rust-like buildup. Outside, the patio and fencing area had broken and rotted railings, exposed rusted nails, fallen palm fronds, and overgrown vegetation, and the Administrator acknowledged the area was not safe for residents. Housekeeping and maintenance staff described daily cleaning and a work-order process, but the Maintenance Director reported being unaware of many of the observed issues, and the DON confirmed there was no specific environmental cleaning policy despite job descriptions and a general policy requiring a safe, sanitary, and comfortable environment.

Failure to Maintain Adequate Hot Water Temperatures at Resident Hand Sinks
D
F0584 F584: Honor the resident's right to a safe, clean, comfortable and homelike environment, including but not limited to receiving treatment and supports for daily living safely.
Short Summary

A resident reported that bedroom hand sinks did not provide warm water, requiring handwashing with cold water and causing discomfort. Surveyors observed that in two rooms, the hot water remained cold despite running for several minutes, and thermometer readings at shared hand sinks showed temperatures in the 70°F range on the hot side, below the facility’s stated 100–110°F expectation. Further observation with the MD revealed that in one room the hot and cold valves were transposed, with hot water only available from the cold side. The MD, ADM, and DON each stated the MD was responsible for monitoring and maintaining water temperatures, but none were aware of recent issues, and the DON did not know the required temperature range. The facility’s maintenance request policy was requested twice but was not provided.

Inadequate Shower Function and Hot Water Temperatures
E
F0584 F584: Honor the resident's right to a safe, clean, comfortable and homelike environment, including but not limited to receiving treatment and supports for daily living safely.
Short Summary

Inadequate Shower Function and Hot Water Temperatures: The facility failed to maintain a functioning shower in the Magnolia unit and failed to keep shower and room sink water temperatures within the expected range. A resident reported delayed showers and inconsistent warm water, while staff confirmed residents were using showers on another hall because the Magnolia shower was out of service and water pressure was poor. Observations and log review showed repeated low hot water readings in Magnolia rooms and showers, and the Wildflower shower also measured below the facility's temperature range.

Failure to Maintain Clean, Safe, and Homelike Environment Throughout Facility
E
F0584 F584: Honor the resident's right to a safe, clean, comfortable and homelike environment, including but not limited to receiving treatment and supports for daily living safely.
Short Summary

The facility failed to maintain a clean, safe, and homelike environment in multiple resident rooms, shower rooms, and common areas. Surveyors observed shower rooms with broken and missing tiles, jagged holes, dark residue in grout, and hair and brown matter in drains. A resident’s dinner tray with food remained on the bed the next morning, and several rooms had wall damage, exposed metal bars near a commode, missing bathroom doors, and vents coated with thick gray buildup. The dining room and hall ceilings had cobwebs and dirty vents, and the kitchen ceiling, pipes, and vents were covered with thick, gray, fuzzy material. Staff, including the Maintenance Supervisor and Administrator, acknowledged that these areas should have been repaired or cleaned and that some surfaces were not included in the cleaning schedule.

Failure to Control Excessive TV Noise Affecting Nearby Residents
D
F0584 F584: Honor the resident's right to a safe, clean, comfortable and homelike environment, including but not limited to receiving treatment and supports for daily living safely.
Short Summary

A resident with dementia and behavioral disturbances consistently kept her TV volume excessively loud, to the point it could be heard from the nurse’s station and the end of the hallway, disturbing nearby residents who reported difficulty sleeping and ongoing disruption. Multiple residents stated that the loud TV had been a problem for some time, especially at night, and one reported needing headphones to block the noise. Staff, including an LPN and a CNA, confirmed frequent complaints, noted that the resident became verbally aggressive when asked to lower the volume, and reported that she insisted on keeping her door open and held the remote to prevent staff from adjusting the sound, despite a care plan indicating an agreed-upon lower volume level.
