Summary

The facility failed to follow physician orders regarding the notification of blood glucose levels outside of set parameters for two residents with diabetes. For one resident with a history of Parkinson's Disease, kidney stones, and Type 2 Diabetes Mellitus with Diabetic Polyneuropathy, physician orders required staff to notify the physician if blood sugar was less than 60 or greater than 400, and to call the primary care provider for certain sliding scale insulin results. Blood sugar readings above 400 were documented on three occasions, but there was no evidence in the progress notes that the physician was notified as required. Facility policy also directed staff to notify the physician for blood glucose greater than 450 unless otherwise directed, but confusion among nursing staff led to missed notifications when readings were between 400 and 450. For another resident with diabetes receiving insulin injections per sliding scale, physician orders required staff to call the physician for blood sugar results over 450.
