The facility failed to ensure that the arbitration agreement was reviewed and explained in a form, manner, and/or language understood by the residents or their legal representatives for three sampled residents. This deficiency was identified for residents who were severely cognitively impaired and included those with diagnoses such as non-Alzheimer's dementia and Alzheimer's dementia.
