The facility failed to implement proper infection control practices during a COVID-19 outbreak, affecting both residents and staff. Eight rooms with COVID-19 positive residents had open doors, contrary to the facility's policy and CDC guidelines, which require doors to be closed to prevent the spread of infection. Furthermore, the facility did not report the COVID-19 outbreak to the California Department of Public Health (CDPH) as required, indicating a lack of awareness among the administration about reporting obligations during an outbreak.
