The facility failed to report an incident of alleged sexual abuse involving a resident, identified as R110, to the State Agency and the Administrator as required. Plan Of Correction

The DON/designee conducted a 2-week look back of incident reports and grievance reports to ensure any abuse allegations made were reported to the state survey agency as required. Audits will be done weekly x 4 weeks then monthly x 2 months. Results of these audits will be submitted to the quality assurance committee to determine if further action is needed.
