Summary

The facility failed to provide adequate supervision and a safe environment for a cognitively impaired resident who was at risk of elopement. The resident, who had a history of dementia with behavioral disturbances, was able to exit the nursing unit through an unsecured door and was later found in the attic. This incident occurred despite the resident being identified as an elopement risk and having a care plan in place that included interventions such as 15-minute checks and transfer to a locked unit.
