Summary

The facility failed to maintain proper infection control practices during medication administration and wound care. During medication administration, an LPN did not perform hand hygiene between residents. After administering medications to one resident, the LPN adjusted another resident's blanket and hair without sanitizing her hands. She then accessed the medication cart and handled medical equipment without performing hand hygiene. The LPN only performed hand hygiene after completing several tasks, which was confirmed as a lapse in protocol by the Infection Preventionist and the Director of Nursing. In another instance, two LPNs failed to don the appropriate Personal Protective Equipment (PPE) while performing wound care on a resident with a sacral pressure ulcer.
