The facility failed to ensure proper communication and documentation regarding the use of prosthetic devices for a resident with a history of cerebrovascular accident and amputations. The resident, who was cognitively intact, expressed discomfort with the prosthetic devices, leading to frequent refusals to wear them. Despite these refusals being documented in the nursing assistant flow sheets, there was no corresponding documentation in the nursing notes, and the physical therapy department was not informed of the discomfort.
