Summary

The facility failed to ensure that a Registered Nurse (RN) completed and documented an assessment for a resident who experienced a change in condition. The resident, who had a history of interstitial pulmonary disease, dysphagia with gastrostomy status, and traumatic brain injury, showed signs of respiratory distress on multiple occasions. Despite being notified of the resident's condition changes on specific dates, the RN supervisors did not perform or document the necessary assessments, including lung assessments and vital signs. This lack of documentation and assessment occurred even though the resident had a history of respiratory distress and aspiration.
