In some cases, the agreement was signed days or even weeks after admission, and in one instance, it was signed by a cognitively impaired resident instead of their representative. Interviews with staff indicated confusion and uncertainty about who was responsible for reviewing and completing the admission paperwork, which includes the bed hold policy. Staff interviews further highlighted a lack of clarity and consistency in the process of reviewing and explaining the admission paperwork. Staff members, including a Licensed Practical Nurse, Medical Records personnel, and the Director of Nursing, provided varying accounts of who should be responsible for this task. The Administrator expected the paperwork to be reviewed and completed within 24-72 hours of admission, but this expectation was not met, leading to residents and their representatives being uninformed about their bed hold rights.
