For Resident #46, who was admitted with vascular dementia and other mental health conditions, the facility failed to ensure that a signed advance directive consent form was available in the medical record. Although the resident's care plan and physician's orders indicated a DNR status, the signed consent form could not be located in the chart. The DNS was uncertain if there had been a signed advance directive prior to the surveyor's inquiry. The facility's policy required that advance directives be completed and placed in the medical record upon admission, but this was not adhered to, resulting in a lack of documentation to support the resident's end-of-life care preferences.
