Record review showed the resident had a history of stroke and repeated unspecified conditions, with a recent Quarterly MDS indicating moderate cognitive impairment (BIMS score of 10). The facility policy stated that if a resident refuses ADL assistance, staff should ensure the resident is safe, re-approach, and notify the nurse or manager if refusal continues.
