Summary

The facility failed to implement care plans for three residents, leading to deficiencies in care. For Resident #9, the care plan required a securement device for a suprapubic catheter to prevent pulling on the skin. However, an observation revealed that the catheter was stretched and pulling taut on the skin, and no securement device was in place.
