Summary

The facility failed to fully complete the annual comprehensive Minimum Data Set (MDS) for a resident, identified as R45, by not completing documentation analysis for triggered care areas. R45's electronic medical record documented diagnoses including cerebral infarction, hemiplegia, hemiparesis, and a pressure ulcer.
