Summary

The facility failed to adhere to the planned menu for a resident on a pureed diet, as well as failed to have a planned menu for residents on pureed and mechanically altered diets. Specifically, Resident #19, who has diagnoses including dementia, dysphagia, and muscle weakness, was served only mashed potatoes and a beverage for lunch, instead of the menu items listed for that day. The resident's physician had ordered a pureed texture diet with slightly thick liquids, but the menu for the lunch meal included items that were not suitable for her dietary needs. Additionally, the facility's menus for September 2024 did not include specific plans for residents on soft and bite-sized, minced and moist, or pureed diets. The dietitian and diet tech confirmed that there were 11 residents on these diets, but the facility only had a regular diet menu.
