Summary

The facility failed to provide adequate supervision and a safe environment for a resident, resulting in a fall from an elevated bed. The resident, who was moderately cognitively impaired and dependent on staff for bed mobility and toileting, was receiving incontinence care from a nursing assistant. During the care process, the nursing assistant turned away from the resident to retrieve clean bed sheets, leaving the resident unattended on an elevated bed.
