The facility's policies, including the Respiratory Protection Program and Admission Criteria, were not followed as required. The Respiratory Protection Program mandates fit testing for respirators to ensure proper use and protection, which was not consistently applied. The Admission Criteria policy requires verification of consents for treatments, including vaccinations, which was not documented for the two residents. These deficiencies in following established protocols had the potential to increase the spread of infection within the facility.
