Summary

The facility failed to immediately initiate emergency medical services (EMS) for a resident who was found unresponsive, pulseless, and not breathing, despite being a full code. The incident occurred when a nurse discovered the resident in this state and began cardiopulmonary resuscitation (CPR) while calling out for assistance. However, there was a delay in additional staff arriving to help, and no one immediately called 911.
