The facility staff failed to consistently document the Activities of Daily Living (ADL) status for three residents, as required by the facility's policy and the Certified Nursing Assistant (CNA) job description. Resident #2, who was admitted with severe impairments and required substantial assistance, had multiple days in July 2024 where there was no documentation of bed mobility, specifically turning and positioning, across all shifts.
