Summary

The facility failed to incorporate the recommendations from the Preadmission Screening and Resident Review (PASRR) Level II Determinations into the assessment, care planning, and transitions of care for three residents. Resident #14, who was severely cognitively impaired, had recommendations for medication education, training in ADLs, training in independent living skills, structured leisure activities, and an evaluation for a diagnosis of dementia.
